Overview
PPO Plus includes the First Choice Health, First Health network, Cigna HealthcareSM PPO Network 1and Kaiser Permanente providers, offering employees living outside of the Kaiser Permanente NW service area nationwide options for care.
PPO Provider Network
Members are served by First Choice Health, First Health Network, Cigna Healthcare PPO Network and Kaiser Permanente providers. Their copay, coinsurance, and deductible are at the same level, regardless of which PPO provider or PPO facility they choose. This network provides care at the lowest out-of-pocket costs.
Non-participating Providers
Members can receive services from any licensed provider or facility they choose (except those already included in the PPO provider network.)
