Dual Choice PPO helps make it more convenient to find doctors and facilities for your care.

In-Network

In Oregon and Washington, visit the Dual Choice PPO Provider Directory.
(This is a combined provider and facility directory that contains Kaiser Permanente, direct contracted, First Choice, and network pharmacies.)

In other Kaiser Permanente States (CA, CO, GA, HI, MD, VA, and DC) to find network providers and facilities, visit the First Health Network.

In all other states to find network providers, and facilities, visit the Cigna HealthcareSM PPO Network1 provider directory.

Kaiser Permanente

  • Access any Kaiser Permanente, affiliated hospital, or pharmacy in Oregon and Washington, all within the integrated delivery system.
  • To find providers, locations, and pharmacies, visit the Dual Choice PPO Provider Directory. or call Customer Service at 1-866-616-0047 (TTY 711), Monday through Friday, 8 a.m. to 6 p.m., Pacific time.

First Choice Health, First Health Network, and Cigna Healthcare PPO Network

  • No referral is needed for office visits to In-Network providers or specialists.
  • Prior authorization may be needed for some services and procedures. Your First Choice Health and First Health Network Providers must call 1-855-281-1840 (TTY 711) and Cigna PPO Network providers must call 1-888-831-0761 for prior authorization. Failure to get prior authorization may result in greater out-of-pocket cost to you.
  • It is always a good idea to call the provider’s office to confirm that they’re In-Network.

Lower cost shares with enhanced benefits

Some in-network providers, including Kaiser Permanente, have lower cost shares for primary care, urgent care, specialty care, routine eye exam visits, and mental health and chemical dependency outpatient services.  This is referred to as an enhanced benefit.

In-network providers who offer Enhanced Benefits are identified with an asterisk (*) in the provider directory.

Can I nominate a physician to become a First Choice Health, First Health Network, and Cigna Healthcare PPO Network Provider?

Yes. If you were seeing a doctor who is not a member of the First Choice Health, First Health Network, or Cigna Healthcare PPO Network, you can nominate that doctor to join our network. Please keep in mind your provider may choose not to be contracted.

To nominate a provider, call 1-866-616-0047  (TTY 711) .

Review can take up to six months. Whether a physician or practitioner is accepted depends on a number of considerations, including the provider’s interest in joining, so a nomination doesn’t guarantee acceptance.

Dual Choice Network Provider Selection Criteria

Kaiser Foundation Health Plan of the Northwest offers several networks that include the Kaiser Permanente integrated health care delivery system. The networks include physicians, specialists, and facilities that provide services to individuals and groups covered under Kaiser Permanente plans. The primary contracted provider groups and facilities that participate include:

 

  • Northwest Permanente, P.C., physicians and surgeons
  • Facilities owned and operated by Kaiser Foundation Hospitals (hospitals, Kaiser Permanente medical offices, and Kaiser Permanente pharmacies)
  • The Portland Clinic
  • The CHP Group
  • Orchid Clinic

The Oregon Dual Choice network is composed of the Kaiser Permanente integrated health care delivery system (as described above) and most First Choice Health providers and facilities in Oregon and Washington.

For members enrolled with an Oregon employer, the Oregon Dual Choice network includes:

  • Kaiser Permanente providers, Kaiser Permanente pharmacies, MedImpact pharmacies, and other in-network providers who are either employed by us or contract with us to provide covered services for members enrolled in this Dual Choice PPO™ plan
  • First Choice Health Network providers in Oregon and Washington
  • First Health Network providers in California, Colorado, Georgia, Hawaii, Maryland, Virginia, and the District of Columbia
  • Cigna HealthcareSM PPO Network1 providers in all other states

The Oregon Dual Choice network is designed to offer access to a broad provider network that includes both Kaiser Permanente providers and a nationwide network at the in-network benefit level. Members can also access out-of-network providers and facilities, although out-of-pocket expenses are generally higher than the in-network provider options.

1. The Cigna Healthcare PPO Network refers to the health care providers (doctors, hospitals, specialists) contracted as part of the Cigna Healthcare PPO for Shared Administration.

Out-of-Network

Choose any licensed provider anywhere for care.

You can work directly with any licensed provider or facility anywhere.
No referral is needed for office visits to Out-of-Network Providers or specialists. Some services are accessed through referral and prior authorization. You or your authorized representative must obtain prior authorization by calling 1-855-281-1840  (TTY 711).

The provider you choose affects your copays, coinsurance and deductibles.  You’ll generally have the lowest costs when you get care from a select group of in-network providers, higher costs when you get care from First Choice Health and First Health Network providers, and the highest costs when you get care from out-of-network providers.
